Nursing Home Transition (NHT) is the process of assisting people to move from a nursing home back to the community, if that is what they desire. NHT begins with identifying nursing facility residents who wish to return to the community. The program
supports potential consumers by coordinating services associated with assisting them to remain in the community.
Services are coordinated by the Nursing Home Transition Coordinator under the direction of the consumer. A Nursing Home Transition Coordinator is available to meet with any nursing facility resident who wishes to discuss the option of returning to the community.
Eligibility
Eligibility for transition services will be determined through a collaborative effort with the consumer, nursing facility, Nursing Home Transition Coordinator, and other identified parties.
Nursing Home Transition offers the following:
- Support and guidance from a Nursing Home Transition Coordinator
- Information and Assistance
- Connection to home- and community-based services and other supports such as transportation, personal assistance, peer support, assistive technology, and home modifications
- Resources to assist with items such as security deposits, household items, adaptive equipment, utilities, and some other necessary expenses to establish a basic living arrangement and to move into that arrangement
