This program provides group activities in a supportive environment for individuals with physical, cognitive or psychosocial limitations who are not capable of full-time independent living.
Eligibility
- Adults aged 60 or older whose physical and/or mental status affects their ability to meet their daily needs are eligible.
- The program serves individuals who are ineligible for Medical Assistance Long-Term Services and Supports.

Once we receive the form, staff will call the person needing help to gather more information and start the process.
